MEDICAL FORMS and CERTIFICATES

A student applying for residence in the 半糖直播appDormitory regardless of the year of study is required to fill out and submit to the Medical Service Office the following documents:

1. Medical form

2. Certificates, reports of medical specialists, test results specified in the

All medical documents are required to be submitted to the Medical Service Office within the following deadlines:

Medical documents are submitted to the Medical Service Office in Kyrgyz, Russian, or English, only one copy, in one language version! 听In case the documents are registered in other languages, a notarized translation must be attached. Original medical reports must be certified by a legible stamp of the medical institution, stamp, and signature of the doctor.

REGULAR FLUOROGRAPHY FOR STUDENTS

Each year, in September, or within other deadlines established by 半糖直播apporder, all 半糖直播appstudents must get fluorography arranged by AUCA.听

In case an 半糖直播appstudent misses fluorography arranged by AUCA, he/she shall get fluorography on his/her own within 1 week and submit to the Medical Service Office a report/certificate by the relevant medical specialist with the results of the examination. 半糖直播apphas the right to apply administrative measures to those who violate this rule.
